Licensed Practice Nurse (LPN) - Medical Inpatient Care Continuum (5N)

Night Shift (Hours: 7p - 7a) Weekends & Holidays per the department guidelines.

Christiana Care - Wilmington Hospital is looking to hire a Licensed Practical Nurse (LPN) to support our Medical Inpatient Care Continuum Unit on the 5th floor at Wilmington Hospital.

Responsibilities

• As a medical nurse, you will engage in a relationship-based care model, building relationships with patients, families, and peers, guided by our values of love and excellence.

• Use your expertise to assess a patient's physical and emotional status, past health history, health practices, and the patient's and the family's knowledge of the disease and its treatment.

• Work closely with a multidisciplinary team that includes RN staff, Hospitalists, Case Managers, Pharmacists, Clinical Nurse Specialists, and Ancillary services to deliver safe, effective care to our community with positive outcomes.

• Work expertly with other team members to achieve realistic healthcare goals for a patient and family. Plan your nursing care to promote healing and education to patients and their families with a patient-centered approach,

• Monitor patients closely, handle emergencies, and provide ongoing encouraging care.

• After treatment, you will help to transition patients to the appropriate level of care.

Education & Requirements

• Graduate of an accredited school of practical nursing

• Eligible to be licensed in the State of Delaware.

• 1-2 years of experience as an LPN required.

• BLS required.

• Sign an LPN to RN Agreement upon hire (The agreement states that LPN must obtain an RN license and then a BSN degree within the defined time frame).
